• Photo Release
• Child Enrollment and Health Information for Child Care Centers and Type A Homes  (Ohio JFS Form 01234)
1. Complete through Emergency Transportation Authorization on Page 3.
  The Acknowledgement of Policies and Procedures and Signatures sections will be completed at GPCP
  the first day of school.
2. Every space, with the exception noted above, has to be completed even if you need to write n/a.
• Second Adult Statement for Child Care Centers and Type A Homes  (Ohio JFS Form 01238)
1. A separate form must be completed and returned for EACH parent or legal guardian.
• Statement of Nonconviction for Child Care Centers and Type A Homes  (Ohio JFS Form 01328)
1. A separate form must be completed and returned for EACH parent or legal guardian.
2. Parents DO NOT need to be fingerprinted.

In order to assure your child's placement, please complete and return both pages of the Member Contract and above listed forms, along with your first tuition payment to GPCP, Attn: Enrollment, PO Box 292, Granville, OH 43023 no later than June 15, 2011.
 
The Child Medical Statement must be completed and sent to our post office box no later than August 1, 2011. State regulations do not allow for your child to participate in our preschool if the necessary forms are not on file or are incomplete.
